Eating disorder - is a radical rethinking required?
The first description of anorexia nervosa by Gull and Lasegue was 1868, the description of bulimia nervosa by Russell in 1979, the binge-eating disorder was proposed by Spitzer in 1992 as a category. Linked to this are the treatment approaches for eating disorders is relatively young and is closely related to the historical diagnostic categories. New data on epidemiology, course and treatment are now the classic lines in question. The similarities in symptoms, patterns of comorbidity, course and response to treatment is far greater than the differences. The majority of patients change in the course of the disease between the different nosological entities. Hence the need for a trans-diagnostic approach in diagnosis and therapy. The work group led by Professor Fairburn has developed on the basis of these considerations, a treatment approach. Eating disorders occur as isolated problems or are part of complex patterns of mental comorbidity. Hence the question of the extent of the treatment: complex problems require complex therapies? The dialectical-behavioral therapy for eating disorders (DBT-E) with a focus on emotion regulation establishes itself as a new treatment option for patients with eating disorders and comorbidity. The point is common mechanisms between personality disorders and eating disorders. Furthermore, a new theory developed for the psychobiology of eating behavior. Here, the importance of supply to the brain with metabolic energy is at the center (Selfish-brain theory). This theory explains lead as stress and trauma, as well as avoidance behavior to changes in eating behavior and weight. In light of these developments it is inevitable that the treatment of eating disorders will be developed in future again.
